7. A medical practitioner with assistant qualifications is required to provide the original and a copy of the assistant certificate of qualification and assistant certificate of registration, together with a copy of the certificate of registration with the stamp of the medical institution; if there is a record of change in registration, a certificate of first registration of practice is required。
8. A copy of a licence to practise a medical institution on a trial basis。
9. The teaching staff and persons with demonstrated expertise are required to provide originals and copies of certificates of teaching and professional competence issued by the provincial chinese medical administration。

V. Form and criteria for fees for examinations
