On 17 october 2015, china's car-t multi-centre clinical research project was officially launched in tianjin. The project will establish clinical research centres in 18 hospitals and involve more than 40 specialists in clinical research over a period of two years, with the aim of evaluating the safety and clinical effectiveness of car-t technology for the treatment of re-emerging/diffic blood tumours and providing a basis for wider application to clinical treatment。
The incidence of blood tumors is rising every year
Malignant neoplasms are one of the leading causes of death in the world today, and according to the 2014 global cancer report, the number of cancer patients and deaths globally increased alarmingly in 2012, with nearly half of the new cancer cases occurring in asia, most of them in china, which is among the highest in the world. More than 3. 5 million new cases of neoplasm occur annually, more than 6 million people are admitted to hospitals and more than 2. 5 million people die of cancer each year. Of the four malignant neoplasms in the liver, oesophagus, stomach and lungs, china has the highest number of new cases and deaths in the world。

Common blood tumours include leukaemia, lymphoma, multiple osteomenoma, among others, which pose a serious threat to human life and health, rank eighth among the 10 most common malignant tumours, and their incidence increases year by year. Blood tumours are acute, and the treatment is long, expensive and painful, but the cure rate is not ideal, under traditional treatments such as decomposition, chemotherapy and bone marrow transplants. The incidence of leukaemia among children is around four out of 100,000, and it is also one of the worst malignant tumours in children, and the number of children suffering from leukaemia has increased each year in clinical treatment in recent years, with leukaemia becoming the number one killer of tumours among children. Therefore, research into the treatment of blood tumours has become a priority。
Car-t treats cancer with its own immunity
Car-t, which uses the patient's own immune system to fight cancer, has achieved an increasing number of encouraging research results in recent years and has been rated by science as the top 10 scientific breakthroughs in 2013。
The car-t cell, full name chimeric antigen recept. T-cell immunotherapy, embedding antigen receptor t-cell immunotherapy, is based on the idea of using the patient's own immune cell to remove cancer cells and is therefore referred to as cell therapy rather than traditional medication。
At the 56th united states blood year conference in 2014, juno and novartis published the results of their car-t-cell treatment techniques for malignant blood diseases. Among them, the total reduction in treatment of acute lymphocyte leukemia in adults was 89 per cent, and the reduction in treatment of acute lymphocyte leukemia in children was as high as 92 per cent. Comment by a medical expert from the national cancer institute of the united states: “car-t cell therapy is more effective than any other immunotherapy being studied!”
Clinical trials of car-t therapy are being actively conducted around the world, including in the united states, the united kingdom, sweden, china and japan. Of these, the united states has the largest number of trials, with 41 trials, accounting for 74. 5 per cent of clinical trials of the global car-t therapy. The university hospital of pennsylvania, the philadelphia children's hospital, the american cancer research institute (nci), fred hutchinson cancer research centre, the sloane cateline cancer centre and the seattle children's hospital were among the first research institutions to conduct car-t cell therapy. China started late in the car-t field and is a single-centre clinical trial. Emerging biopharmaceutical companies such as juno, kite, celgene, cellectis, traditional pharmaceutical companies such as novara, glaxoschick, pfizer, johnson, etc. Have invested in the development of car-t therapy to facilitate the early clinicalization of this highly promising treatment。
Car-t cell therapy has great potential and will become the most effective treatment for oncology and routine treatment in the future。
The multi-clinical center research project gives hope to patients with blood tumors
This area has developed considerably in a relatively short period of time, and the success of a new treatment depends on its efficacy on the patient. In order to judge the effectiveness of this treatment in a comprehensive manner and to increase the number of patients with blood tumours treated, there is a strong need for joint car-t multi-centre clinical research in china, which requires considerable human and financial support. With support from kunlong, shanghai, a clinical research centre will be set up in 18 hospitals, including the chinese academy of medical sciences, the people's hospital in jiangsu province and the wuhan tozi hospital, with the aim of evaluating the safety and clinical effectiveness of the car-t treatment for relapses/difficult blood tumours and providing a basis for wider application to clinical treatment。
This multi-centre clinical research project lasts for two years, including the start-up phase, the car-t treatment phase for incoming patients, the short-term follow-up phase, the long-term follow-up phase and the clinical data summary phase。
This is the research project with the largest number of research centres and the highest number of cases in the country. In the future, the study will bring evangelization to patients with re-emergence or incurable blood tumours, including those who re-emergence after drug treatment, who are not suitable for the same type of heterogeneity stem cell transplant or who are re-emerging after the same type of heterogeneity stem cell transplant. The tumour load of patients can be removed or reduced, the life period extended and the conditions created for bone marrow transplants for some patients。
The fact that quinlan in shanghai is an autonomous national enterprise has raised the level of specialization in cell therapy in the country. This multi-centre project is designed to reduce the burden on patients while enabling them to benefit from the new technology. The number of hospitals and patients participating in the project far exceeds the number of other well-known pharmacists, both inside and outside the country, and the data available in the experiment better reflect the chinese response to the technology, which will no doubt be a blessing for chinese patients with blood tumours。
